Abstract
A computer based outpatient data system, which is used in a major pediatric teaching hospital, was designed to simplify record keeping in a large decentralized clinic operation and to conserve professional time. Family and case demographic information and clinical data are collected as critical bits (selected and unduplicated input). Data acquisition forms serve as integrated components of the patient's record and as vehicles to enhance patient flow. Error surveillance programs, external reports, and internal reports for administrative purposes are generated at nominal cost. In a period of 21 months, the processing of more than 187,000 forms demonstrates the efficacy of the system.
