Spontaneous Hypoglycemia in Diabetic Patients With Renal Insufficiency

Abstract

We have observed three patients with diabetes mellitus, in whom renal insufficiency was associated with decreased requirements for exogenous insulin or hypoglycemic agents given orally. All three patients, while being managed on dietary therapy alone, became hypoglycemic during periods of decreased food intake. Studies on one subject demonstrated low serum insulin, elevated growth hormone, and normal cortisol levels. It seems probable that impaired hepatic glycogenolysis, glyconeogenesis, or both are responsible for this syndrome.
