Specialist Delivery of Primary Care

To the Editor.—The California Academy of Family Physicians wholeheartedly applauds the publication of the article by Dr Rosenblatt and colleagues1 on the role non-primary care specialists play in providing primary care.

Given the finding of Rosenblatt et al that "most specialists do not assume the generalist role," we find that the accompanying Editorial by Dr Vanselow2 incongruously embellishes the partial ways in which 2 or 3 non–primary care specialties meet the standard of primary care, as established by the study's 3 proxies. The Editorial does not acknowledge the value of primary care specialties and seems at odds with the conclusions of Rosenblatt et al.

The article by Rosenblatt et al is an excellent initial effort in addressing the issue of quantity and quality of primary care delivered by non–primary care specialists. Its publication is especially important given the current demand from patients for direct access to non–primary care specialists. . . . [Full Text of this Article]
