Recognizing that the presence of vancomycin-resistant bacteria in orthopedic patients has been increasing, the American Academy of Orthopaedic Surgeons has issued recommendations on the prophylactic use of antibiotics.

The recommendations ask physicians to reserve vancomycin for treating serious infection with -lactam–resistant organisms or patients with life-threatening allergy to -lactam antimicrobials. Contact isolation for patients with vancomycin-resistant organisms also is recommended. The academy has emphasized the importance of reporting strains of Staphylococcus aureus with reduced sensitivity to vancomycin to state and local health departments and to the Centers for Disease Control and Prevention in Atlanta, Ga, at (404) 639-6413.
