Challenges Facing Family Practice and Primary Care

In Reply: Drs Mundinger and Kane assert the importance of nurse practitioners as primary care clinicians. We wholeheartedly endorse the notion that physicians are not the only primary care clinicians, and acknowledge the valuable role played by nurse practitioners, physician assistants, and other professionals in primary care. However, we believe that arguments about which discipline is the "best" provider of primary care are counterproductive. It is time to move beyond "turf battles" so that all primary care clinicians may collaborate in designing more effective and satisfying primary care systems, drawing on the unique skills of each discipline.

Dr Bicket describes the compelling problems of the uninsured and disadvantaged populations. Although we mentioned the "inclement external political and financial forces raining down on the primary care home," we chose principally to focus on issues more intrinsic to primary care itself.
