Acarbose for Patients With Hypertension and Impaired Glucose Tolerance—Reply

In Reply: We thank Dr Rosenthal for bringing to our attention his finding that acarbose had a significant blood pressure–lowering effect in patients with hypertension and type 2 diabetes.1 We did not claim that we were the first to demonstrate that acarbose could reduce hypertension. We claim that our trial is the first prospective study to find that acarbose can decrease the risk of new cases of cardiovascular disease and new cases of hypertension in patients with IGT.2 As far as we know, that is still true. We also found that acarbose could reduce both systolic and diastolic blood pressure in this group of patients, an observation consistent with Rosenthal's observation in patients with hypertension and diabetes.
