To the Editor: Among the articles in the series Innovations in Primary Care, Dr Bodenheimer and colleagues have addressed issues as diverse as chronic illness management and improving access to primary care. It would have been interesting had Bodenheimer et al discussed the advanced-access model of patient scheduling, which was described in the accompanying article by Dr Murray and colleagues,1 in the context of the other articles in the series. I am concerned about the potential effects of the advanced-access model on health care outcomes, particularly among patients with long-term illness.
In the article discussing the chronic care model,2 the authors described a Danish study of primary care disease management for diabetes.3 As the authors noted, this management program involved regular follow-up visits and resulted in substantially lower glycohemoglobin levels.
