Treatment of Men With Lower Urinary Tract Symptoms and Overactive Bladder—Reply

In Reply: We agree with Dr Chai that overactive bladder likely plays a greater role in urinary urgency and urgency incontinence than does BPH. Regarding the comments of Mr Bell and Dr Amarshi, over-the-counter decongestants and antihistamines were not prohibited, which is the convention in {alpha}-receptor antagonist and antimuscarinic clinical trials. No potential interactions with these products are described in the tamsulosin or tolterodine extended release prescribing information, and we are unaware of any evidence suggesting that their efficacy is compromised by these products. We did conduct medication reconciliation to assess compliance. Unused medication was recorded at each visit or upon early termination. Patients who took at least 80% of their study medication were considered adherent.

We disagree with Drs Young and Subramonian that the issues they raise preclude conclusions about the efficacy of combination therapy in men with lower urinary tract symptoms including overactive bladder. They state that the . . . [Full Text of this Article]
