Adjuvant Radiotherapy After Surgery for Prostate Cancer

To the Editor: Dr Trock and colleagues1 analyzed the effect of salvage treatment after radical prostatectomy on prostate cancer–specific survival. They concluded that "these results could motivate a clinical trial comparing adjuvant with salvage radiotherapy, with prostate cancer–specific survival and overall survival as the primary end points." The recently opened RADICALS (Radiotherapy and Androgen Deprivation in Combination After Local Surgery) trial2 (ISRCTN40814031) is currently addressing this issue.

RADICALS will be a large intergroup phase 3 trial, with 2 randomizations and a pragmatic design. Where there is uncertainty about the need for adjuvant radiotherapy (RT) after radical prostatectomy, patients may enter the RT timing randomization between adjuvant RT and an observation strategy. Patients in the observation strategy will receive early salvage RT in the event of prostate-specific antigen failure.
