Therapy Recommendations for HIV-Associated Neurocognitive Disorders

To the Editor: The report of the 2008 recommendations of the International AIDS Society–USA Panel by Dr Hammer and colleagues1 provides current standards for the complex decision-making around antiretroviral treatment for AIDS. The article discussed when to start treatment in special populations, including persons with or at risk of specific opportunistic diseases and coinfections (hepatitis B and hepatitis C virus), patients with human immunodeficiency virus (HIV)–associated nephropathy, and pregnant women.

However, the article did not consider initiating treatment in relation to HIV-associated neurocognitive disorders, including dementia. I would suggest that future recommendations consider the potential role of central nervous system (CNS) sanctuary for HIV replication.2-4
