To the Editor: The meta-analysis by Dr Soylemez Wiener and colleagues1 assessed the benefits and risk of tight glucose control in critically ill patients. In the Comment section, the authors discussed the quality of glucose control and found that 21% of the studies included did not achieve a mean glucose level within 5 mg/dL of the stated goal. This success rate presumably refers to the intensive treatment group only, although not stated explicitly.
In our view, the evaluation of the success rate of glucose control should not rely on such statements. Other methods, including the time to reach the target and the proportion of values within the assigned range, may add important information for a more accurate assessment of the quality of glucose control. Moreover, the mean glucose values used to calculate the average were either the average of the morning blood glucose values2-4 or the average of every recorded . . . [Full Text of this Article]
