All estimates are based on the covariate distribution from NHANES 1999-2002, the number of deaths in 2000 from US vital statistics data, and the relative risks estimated from National Health and Nutrition Examination Surveys (NHANES) I, NHANES II, NHANES III, or the combined NHANES I, II, and III data set. Error bars indicate 95% confidence intervals.
