Incorrect Footnotes and Omission of Research Group Member in: Chronic Dialysis and Death Among Survivors of Acute Kidney Injury Requiring Dialysis

Incorrect Footnotes and Omission of Research Group Member: In the Original Contribution entitled "Chronic Dialysis and Death Among Survivors of Acute Kidney Injury Requiring Dialysis," published in the September 16, 2009, issue of JAMA (2009;302[11]:1179-1185), on page 1181, in the "a" footnote for Table 1, the word "dates" should have been "data."

On page 1183, Figure 3, a footnote symbol was omitted in the plot and legend. A "b" footnote symbol should have been included in the first column after the "Yes" rows beneath Peripheral arterial disease, Cardiac surgery, and Aortic aneurysm repair. The same symbol should also have preceded the footnote that read "Effect estimates are not shown; models did not converge." This footnote indicates that effect estimates for the adjusted risk of chronic dialysis could not be calculated for individuals with a history of peripheral vascular disease, cardiac surgery during the index hospitalization, and aortic aneurysm repair during the index hospitalization due to the relatively small number of patients in these subgroups.

On page 1185, center column, the name and affiliation of a member of the University of Toronto Acute Kidney Injury Research Group was omitted: C. David Mazer, MD (St Michael's Hospital).
