To the Editors:—
The application of sound surgical principles of drainage to the treatment of chronic recurrent hidradenitis suppurativa is to be encouraged, but it is less often successful than might be thought because of the basic pathology of the disease. That is, when the apocrine glands become the site of chronic infection, intertwining, communicating, and deeply penetrating epithelial lined tracts are produced whose unroofing and exposure is difficult and most often incomplete. It is to be emphasized that in the patients we have treated by surgical excision, almost all have already had multiple surgical incisions and drainages of the type suggested by I Gever. The therapy we advocate is not to be applied to the early and previously untreated case of hidradenitis.
