To the Editor. —
I must point out that the brief report "New Treatment of Barbiturate Dependence" by D. E. Smith, MD and D. R. Wesson, MD (JAMA 213:294, 1970), is in no sense a new form of treatment for barbiturate dependence, nor are any fresh ideas regarding the "abstinence syndrome" presented. The subject was exhaustively treated in The Barbiturate Withdrawal Syndrome by M. H. Wulff in 1959 (Copenhagen, Einar Munksgaard Forlag). Wulff describes adequately the manifestation-withdrawal syndrome and its treatment with phenobarbital substitution in gradually diminishing dosage. It is noted that the dosage of phenobarbital indicated in the report of Smith and Wesson seems unnecessarily high. The technique of Wulff has received wide application in this country, particularly since his monograph appeared as a supplement to the journal, Electroencephalography and Clinical Neurophysiology.
