To the Editor.—
In reference to your recent editorial comment entitled "Automatic Detection and Defibrillation of Lethal Arrhythmias—A New Concept"1, readers might be interested in a recent paper by our group2 which, in a sense, supplements the paper by Mirowski and colleagues.3
In contrast to the work reported by Mirowski et al, our approach involved electrodes implanted between the pectoralis major muscle and the rib cage, detection of fibrillation from the electrocardiographic signal, a quasi-trapezoidal defibrillatory shock, and a system small enough to be completely implanted. Some 73 successful defibrillatory episodes were reported in three dogs with completely implanted systems.
