Ultrasound in Evaluating Renal Masses

To the Editor.—

The recent letter by Dr Judith Everett (235:2813, 1976) does not accurately reflect the use of diagnostic ultrasound for the evaluation of renal masses. It is generally accepted that homogeneous tumors, of which hypernephroma is one, may superficially mimic a cyst pattern. Subtle differences may be noted in "back wall" characteristics as well as internal structure. However, to discover the group of lesions that masquerade as cysts, aspiration is strongly recommended to alleviate the need for arteriography and surgery for benign cystic lesions. The aspirate should be sent for appropriate laboratory testing, including cytologic examination. At the time of aspiration, positive contrast may be instilled into the cyst and radiographic examination of the cyst wall contour performed as an added measure in doubtful cases.
