To the Editor.—
Daniel J. Abramson, MD (235:1423,1976) recommends that at the time of sigmoidoscopy examination a vaginal examination and Papanicolaou smear should be made. He states: "In this area, with an excellent gynecologic service, it was found that very few patients qualified for this study, since most had had a pelvic examination within a six-month period prior to sigmoidoscopy."
In my own experience in this area, with an excellent sigmoidoscopic service, very few patients who come to me for an annual pelvic examination and Papanicolaou smear hav ever had a sigmoidoscopic examination. In other words, it seems that we might do far better to secure this combination of examinations for our patients, rather than the reverse as Dr Abramson has suggested. Our local gastroenterologist recommends proctoscopy every two years in patients more than 40 years of age.
