To the Editor.—
The article by Miller and Griepp, "A Modern View of the Surgical Treatment of Peripheral Arterial Disease" (240:1524, 1978), attempts to stimulate referrals for this form of therapy.
The emphasis of the article is on the low operative morbidity and mortality and the rationality of the procedures, while elaborating on technical surgical details. These are important; however, there was a sparseness of discussion on proof of efficacy—the overriding consideration—by well-controlled and well-designed studies to support this therapeutic modality preferentially.
Until that is done, the "communication gap" described by the authors may persist. This comment should probably exclude aortic aneurysms, since the dismal course of the untreated disease appears to be well documented.
