I am sorry that Dr Jacobsen was quoted incorrectly; however, I was not to blame. Nonetheless, Dr Jacobsen will recall that shortly after the Lancet editorial appeared, I discussed its contents with him and suggested to him that he was misquoted. He agreed with me but thought that the editorial statements were so nearly correct that to challenge them was unnecessary. Several months later, he had second thoughts. My letter to JAMA was written before Dr Jacobsen had his change of mind, and had I known of them I would not have used the particular quote to which he objects, the more so since there is ample evidence in support of my views. . . .
