In Reply.—
There is a general misconception that the PSRO is a faceless, unilateral entity that does "things" such as "criteria selection." In fact, criteria are selected by groups of physicians with expertise in their respective areas. In the study described in our article, a subcommittee of the Colorado Health Care Standards Committee established the criteria. This committee is composed of representatives appointed by their state-wide specialty societies. In the case of obstetrics and gynecology, the gynecologist on the committee was the appointed representative of the Colorado Obstetrics-Gynecology Society.
The criteria described are part of a structure that precludes their application by "bureaucrats." The criteria were used as guidelines by nurse reviewers to screen out cases for physician peer review. Physicians whose cases fell outside the indications were not "castigated."
