To the Editor.—
In the Dec 7, 1984, issue of THE JOURNAL, Dr Dworken1 discussed the management of a patient with abdominal pain and intermittent diarrhea presumably caused by atrophic gastritis.
Because of the increased number of elderly patients who now undergo endoscopy, atrophic gastritis is not an uncommon histologic finding in the gastric biopsy specimens.
However, before starting a regimen of antibiotics for presumed bacterial overgrowth for such patients, I would strongly recommend that lactose intolerance be excluded either by dietary elimination or, preferably, a lactose tolerance test using the patient's symptoms (or lack thereof) as the end point. Because intestinal transit times in the elderly may be slower, symptoms may not develop until several hours after ingestion of the lactose.
