In Reply.—
We agree with Drs Abemayor and Mickel and recognize the potential problems accompanying premature cervical biopsy in older patients. We did not mean to imply that biopsy for these patients should precede or replace a thorough examination for primary head and neck cancer, but rather that this evaluation should begin early in presentation. As noted in our original article, the model we described was developed and tested in a 9- to 25-year-old population. We again must emphasize that it should not be applied to either younger or older patient populations.
