To the Editor.—
The article on caffeine and benign breast disease in the April 26 issue of JAMA1 has serious flaws and is misleading to the medical profession and the public. The article indicates that there are 854 patients with benign breast disease in the study group and claims there is no relationship between caffeine and benign breast disease. In this report, there are 179 patients with fibroadenoma, a tumor that, in our studies, has never indicated a sensitivity to methylxanthines.
In addition, the study group includes 97 patients with no specific histopathology and 46 patients with a diagnosis of granuloma. None of these three conditions, as far as I know, have ever been suspected of being sensitive to methylxanthines.
This means a total of 322 (38% ) out of 854 of the patients really do not belong in a study evaluating the effects of caffeine on benign breast disease.
