In Reply.—
I read the letter of Dr Ertel with interest and appreciate the opportunity to respond to it. At Ford Hospital, we have also seen a case wherein the first manifestation of recurrent Graves' disease after methimazole treatment was a recurrence of the patient's urticaria and angioedema. There was no mention of urticaria or angioedema in the second case Dr Ertel described, but it was important for the relationship with the severe pruritus to be recognized as hyperthyroidism.
At no point in my original response did I emphasize the rarity of the association between urticaria and hyperthyroidism, but I did state that it was uncommon. In fact, as stated, I routinely order tests to assess thyroid function in patients presenting to the allergy clinic with urticaria, especially in young females, in whom thyroid disease is more commonly seen.
