In Reply.—
Even if the treatments being compared are equivalent, randomized allocation must be substituted for any preference the physician might have for a particular treatment in a given patient: hence, "judgment is set aside." The word "substandard" may be a little strong; however, if one of the treatments in the trial emerges as distinctly superior to the others, patients having received the latter have—inadvertently—been given an inferior treatment during the trial.
I assume that physicians explain these two fundamental concepts to patients whom they are encouraging to participate in a clinical trial.
