In Reply.—

We regret that our concern for what we believe to be truly informed consent and a patient's freedom to decide about participation in clinical trials has been misinterpreted by Dr Jaffe as a reflection of "over-whelming cynicism."

Dr Jaffe does "not disagree... that patients should know if the physician is receiving a special bounty for enrolling them in clinical trials." He apparently believes, however, that "special bounties" are not of great size and are seldom paid.

Our professional experiences are similar to those described by Dr Spiro, who is concerned in his important commentary with the "irresistible" opportunity to complete a study in which considerable or lesser sums are paid for each subject.
