To the Editor.—
I read with interest the article by Zuger et al1 entitled "Pentamidine-Associated Fatal Acute Pancreatitis." The authors described a case of severe hemorrhagic pancreatitis that developed in a patient with the acquired immunodeficiency syndrome during pentamidine isethionate treatment for Pneumocystis carinii pneumonia. According to this report, "The pancreas was grossly necrotic; microscopic examination confirmed massive necrosis of both islet and nonislet tissue, surrounded by an inflammatory cell infiltrate. Islet cells within the nonnecrotic areas of the pancreas appeared normal."
Study.—
I have found a diabetogenic effect of pentamidine in mice.2 Pentamidine mesylate (Lomidine) was injected in a daily dose of 4 mg/kg of body weight intramuscularly for ten days in the study group. The control group received saline solution intramuscularly. On the 11th day, blood samples were obtained and the pancreata of the mice were removed.
