In Reply.
—The physical examination of the patient revealed a normal physiological split of the second heart sound. The second heart sound did not have a fixed split. A two-dimensional echocardiogram was performed with injection of saline into a peripheral vein. There was no evidence of a right-to-left shunt from the right atrium to the left atrium.
Hydrogen was not introduced during the right-sided catheterization and blood samples were not obtained to evaluate a possible step-up in the oxygen saturation.
