To the Editor.—
Manninen et al1 prove again that lowering the non-high-density lipoprotein cholesterol level lowers the risk of coronary disease.
What is missing is evidence that they have also lowered all-cause mortality. Since the treated subjects in this study experienced only two fewer fatal heart attacks, it would take only two noncoronary deaths to invalidate their conclusion that gemfibrozil was "effective in the primary prevention of CHD."
