To the Editor. —
The review article entitled "Chronic Cor Pulmonale: Etiology and Management"1 is thorough in its discussion but incomplete in its scope. Certainly the subject of chronic cor pulmonale should include a discussion of at least three other common causes of this process: chronic left ventricular failure, mitral valve stenosis, and chronic mountain sickness. Although the mechanism of chronic cor pulmonale in these clinical states varies significantly, the pathophysiology is similar to that described for some of the other processes mentioned in the article. While the authors may have intentionally omitted these clinical syndromes, I think that a thorough discussion of the problem of chronic cor pulmonale demands their inclusion.
