To the Editor.

—Two articles1,2 and a related editorial3 in a recent issue of JAMA discussed the cost of medical care in Canada and the cost impact of the number of physicians produced by our training systems.

It is most surprising that in these three articles, neither the differences between Canada and the United States in respect to the medical care needs of indigent patients or the needs within the United States for expanding the care of underserved groups were considered.

Clearly, the employment levels of the two countries are major factors in the structure of medical care systems and should not be overlooked in these types of evaluations. . . . [Full Text PDF of this Article]
