—The May 15 JAMA contained many comprehensive proposals for responsible health care reform in this country. The editorial noted the adverse disparity between types of physicians. Two subsequent articles in the Caring for the Uninsured and Underinsured column have focused on production of primary care physicians.^1,2 The Editor believes that we must increase the number and percentage of primary care physicians by the following measures:

1. Sharply and promptly increasing the number of MD graduates entering primary care.
   
2. Providing meaningful fiscal incentives and disincentives to effect and maintain the shift by widespread application of the Resource-Based Relative Value Scale.3
   
3. Developing retraining opportunities and pathways for many (?? say?? 100 000 or so) of the oversupplied specialists to become competent practicing primary care physicians
