To the Editor.

—Guidelines for CPR and emergency cardiac care are nicely presented in the October 28, 1992, issue.1 However, it is unfortunate that organized medicine has ignored the potential role of screening of the high-risk population. Rather than emphasis on the management of the emergency crisis, there should be consideration for early identification of silent ischemia and risk stratification so that appropriate therapy might avoid death or a myocardial infarction. It might be helpful to adopt the concept of the American Cancer Society, which promotes screening and the early detection of malignancies to engender broader options and lower costs. . . .
