To the Editor.

—I was intrigued to read that a singlesource financing system might suffer from "provider capture."¹ What is the meaning of this undefined yet alarming expression?

Could physicians and nurses become sullen prisoners, marching glumly to ward rounds under armed guard? Or did Dr Saltman envisage guerrilla raids by outraged health workers, to hijack the bureaucrats who guard this vast pile of cash?

Either way, it looks as if interesting times might lie ahead.
