To the Editor.
— After reading the Editorial by Dr Johns,1I am stirred to write to JAMA to recall a proposal JAMA chose not to publish nearly 20 years ago. Reflecting on the increasing trend for medical school graduates to enter residency training immediately after completing their internship year, and an alarming trend for fewer primary care physicians (even then), I suggested consideration be given to requiring graduating physicians to have 2 years of general practice experience before being accepted into specialty training programs. The thought was that choosing a specialty before having any clinical training in medical school was almost becoming a necessity to compete for postgraduate programs.
Now it seems that the choice needs to be made before schooling even begins. Two years of general practice experience would have furnished the individual physician with valuable additional knowledge, and at the same time, provided a renewable source
