—We appreciate Dr Bartel's comments and agree that the dysmotility may be related to autonomic dysfunction.
In contrast to what Dr Cook and Dr Epstein have stated, patients and controls were enrolled exactly as described in the article. Our clinical observations stimulated interest and contributed to the questionnaire's development. The patients were identified and studied (between January 1992 and January 1993) before any questionnaire results were available. Controls were obtained from a referral population of more than 3000 patients during a 3-year period and "winnowed" down with the same selection criteria as patients. Our relationship with CATS and D. M. Dunleavy, Esq, has been questioned. Because two of the patients studied in our article are children of the director of this organization (studied before its formation) and preliminary results had been reported in abstract form,1 CATS was aware of some of our observations.
