To the Editor.

—While the Review by Dr Baker and colleagues¹ was educational, I was surprised that they did not mention the role of low-dose amiodarone in severe heart failure. It has proven to be effective and reliable therapy, reducing mortality and hospital admission independently of the presence of complex ventricular arrhythmia.²

The SOLVD Investigators³ say that there is a need for further treatments to reduce mortality in patients with heart failure, by action on more than one mechanism of death. Recent studies^2,4,5 show that the decrease in mortality obtained is greater than would be expected from amiodarone from only an antiarrhythmic effect, and the beneficial effects of amiodarone on mortality and cardiac function are in addition to those that may have accrued from full conventional treatment.^2,4,5 . . .
