To the Editor.— The article by Nair et al (1980;243:1552), "Acute Myocardial Infarction in Idiopathic Hypertrophic Subaortic Stenosis," describes an unusual case of acute myocardial infarction in a young patient with idiopathic hypertrophic subaortic stenosis that the authors explain was related to strenuous exertion during cold temperature. Although the patient was described as receiving propranolol hydrochloride, 80 mg daily, in July 1976, no mention of present therapy or, more important, cessation of therapy is mentioned. Could this patient be another example of abrupt propranolol withdrawal resulting in acute myocardial infarction?1,2
