ELEVATION of creatine kinase (CK) isoenzymes in patients with malignancies has been reported. The isoenzyme BB is commonly present in stage D prostate cancer but has also been reported in lung cancer of all histological types. This ectopic CK-BB may migrate to atypical positions on gel electrophoresis, depending on whether it forms complexes with immunoglobulins or other macromolecules. By contrast, the MB isoenzyme is rarely produced ectopically from carcinoma.1 The patient described herein had a metastatic adenocarcinoma and a CK electrophoretic pattern on cellulose acetate that showed a normal MM peak with a larger peak anodal to the MM peak, in the position normally occupied by the CK-MB isoenzyme.
